Support Services for Older People in Whanganui

Your Way | Kia Roha can coordinate support services for people who have age-related needs and live in Whanganui and are over 65 years old.

Te Whatu Ora (Health New Zealand) will refer you to Your Way | Kia Roha.  After we receive your referral, we will contact you. We will then work with you to find and organise supports that suit you.

Support ideas could include:

  • support from whānau and friends
  • local networks
  • aged-care organisations
  • home support
  • rest home care
  • other government-funded supports

Who is eligible?

If you’ve been through the Discovery process with Your Way | Kia Roha, we’ll work with you to find and organise supports that suit you.

Your Way | Kia Roha can coordinate services for people over age 65, who have an age-related disability, live in Whanganui and have been referred by Te Whatu Ora (Health New Zealand).

To get started with any of our Connections & Funding services, simply get in touch with us. We’ll guide you through the next steps.

Staying in touch

If your needs or circumstances change at any time, let us know so we can talk with you and review your plan.

Please tell us about any changes which might affect your disability support needs, including:

  • going to the hospital
  • significant changes in health and disability
  • changing your contact details
  • moving house

Even if we don’t hear from you, we will make sure that you are re-assessed every three to five years. We’ll get in touch with you to arrange that meeting when it is time.

You can also ask to be re-assessed earlier, particularly if there are any big changes in your life.
