Supporting ACC Clients with Disabilities
Living My Life are specialised services to support ACC clients with a disability to have choice and control over where they go, what they do, where they live, and who with.
ACC contracts Your Way | Kia Roha to work with people in the Hutt Valley to:
- Think about how they want to live their life and develop a visual map that the person can use to let others know what is important to them (Facilitated Pathway Map).
- Understand the range of programmes and services available to them and make decisions about how they would like to be part of their community (Independent Facilitation).
