How We Work With You

Your Way l Kia Roha’s Connections & Funding teams support disabled people to live the best life they can, providing a Living Well planning and connection service (previously NASC and Local Area Coordination). 

Whatever Living Well Looks Like, We’re Here to Make it a Reality  

Your Way | Kia Roha Connections & Funding teams know that living a good life is important. We’re here to understand what living well means for you. We’ll help you plan, and connect you to the people, resources, support, and information you need to live a good, or better life.

Start by Connecting with Us 

The first step is to connect with our team. You can phone us, or read our website, check if you’re eligible for our services, and make a referral.

Completing a referral form is only required if you live in Tairāwhiti, Taranaki, Whanganui, Hutt and Otago and Southland. 

If you’re in the Bay of Plenty and Lakes area you don’t need a formal referral to us. Contact us on 0800 758 700, ask to speak with our Bay of Plenty team and you’ll be connected.

Our team love helping people with disabilities, and their family | whānau and carers, to get access to the support and funding they need. 

What We Do  

If you’ve sent a referral or are referred to us, we’ll check that you’re eligible for support, and be in touch within five working days to talk through the next steps.

If you’re not eligible, we will connect you with another service, or give you guidance on where to get help.

Tō Oranga Pai | Living Well Plan

If you’re eligible, we’ll talk with you about developing a Tō Oranga Pai | Living Well Plan. The Plan identifies what it means for you to live well and what you want to do to achieve your goals. These goals may be large or small. 

Planning is led by you, at the place you want to meet, the pace that you want to do it, and with the people you choose (whānau / family, friend, advocate or caregivers). The Plan is yours and is owned by you.

We can support your planning by arranging cultural support or interpreter services, including a NZSL interpreter. 

You may want to complete your Plan on your own, or with whānau before contacting us. Go to Tō Oranga Pai | Living Well Plan on our website to download a copy.

Connections & Funding 

Once we know what you want to lead a good or better life, we will connect you with the people, support, or resources to achieve your goals.

Support might include:     

  • Formal or informal support from family / whānau and friends
  • Personal care or home help
  • Individualised funding
  • Local networks, community or disability organisations
  • Funded Family Care
  • Home and community support, including respite and carer support
  • Supported living
  • Residential support

Support may also be information about local services, community groups and activities you can join. It may be equipment or ideas about new hobbies, work or places to meet new friends. It might be conversations about goal setting and dreaming big about your life.

Some support needs funding, and we will explore the best options for you. If the funding isn’t available to achieve all your goals, we will talk with you about what’s most important so you can start getting what you need to live well.

Staying in Touch

If life changes for you after you’ve made a Plan, and your support needs change, contact us so we can help. We will also keep in touch to check your Plan is going the way you wanted.
